Getting Ready for your First Appointment:

For your convenience, a printable version of our New Patient Packet is available online.

You may print and complete the New Patient Packet prior to your visit and bring the completed forms with you when you come in. 

If you have problems downloading the new patient packet,  right click on the above link and save the document to your computer. Then print the packet directly from your computer.

If you are not able to complete the New Patient Packet before your visit, please allow at least 30 minutes for completion in the office before you see the doctor.

Find out what the office visit co-pay is for your particular insurance company and be prepared to pay that amount at the time of your visit.  

Payment can be in the form of cash, personal check or credit card. We accept MasterCard, Visa, Discover and American Express.

Be sure to have your insurance card(s) and driver license with you.

Please bring all medications you are taking with you.
