Health Information Management (HIM) is responsible for the management of medical records at Brookwood Medical Center.  The HIM department is located on the second floor of the hospital and is open to the public from 8:00am to 4:30pm Monday through Friday. 

Release of Information
Release of patient information requires a signed written authorization by the patient that meets privacy requirement regulations.  Copies of medical records are available upon written request with valid identification. Pursuant to Alabama law, there is a charge for photocopies of medical records. $1.00 per page for the first 25 pages copied, and $0.50 per page for each page copied thereafter. There is no charge for records to be faxed to the physician.

Birth Certificates

Birth certificate information is collected by certified hospital personnel following delivery and prior to discharge.  This information is submitted to the Alabama Department of Public Health. Birth certificates can be purchased from the state at a charge of $15 for the first certificate and $6 thereafter.

Death Certificates

Death certificates are initiated by the hospital and submitted to the appropriate funeral home. It is the responsibility of the funeral home to submit the death certificate to the proper state authority.
