Hours of Operation/Appointments

Brookwood Maternal Fetal Medicine's regular office hours are Monday-Friday from 8:00 am to 4:30 pm. Appointments are necessary.

Location

Birmingham - Brookwood Maternal Fetal Medicine is located in Suite 604 of the Brookwood Women's Medical Plaza. Parking is available in the yellow zone of the visitor parking deck for those visiting the Women's Medical Plaza. Valet parking is also available at the entrance of the Women's Medical Plaza.

Coming soon to Huntsville, Alabama. The office will be located at 401 Lowell Drive Southeast, Suite 24.

Insurance Policy

Our practice participates with many insurance plans and we are adding new plans at all times. Participation means the practice will file claims to these insurance companies and will make the proper fee discounts according to our contract with that company. We will verify insurability with most insurance plans before your are seen. Payment of any deductible and co-pay will be expected at the time of visit. Secondary insurance will be filed as a courtesy.

Payment Policy

Payment is expected at the time service is rendered. Your insurance company requires you to pay your co-pay at the time of service.

The cost of billing, which we are unable to control, is constantly increasing. In order that we may keep our fees as low as possible, we are trying to eliminate billing. If you request to receive a bill, we will add an additional $5.00 for billing charges.

An account which is past due and for which no payment arrangements have been made will be forwarded to a collection agency. Collection agency fees will be the responsibility of the patient.

There is a $30.00 charge for all returned checks.

You may print and complete the forms prior to your visit. Bring the completed forms with you to your first appointment. If you are not able to complete the forms before your visit, please allow at least 30 minutes for completion in the office before you see the doctor.
